Columbus Regional Hospital is committed to providing excellent care which includes speedy and courteous registration. We have improved our services to include on-line pre-registration. When you arrive at the hospital all of your information will be in our system which will save you time.

Please complete this form at least three days (72 hours) before your visit to allow adequate time for processing your information. If your appointment is within the next three days, please use phone pre-registration:

Phone registration is available Monday-Friday from 9 am- 8 pm.

• When you arrive at the hospital, please go to the front desk to check-in and confirm your registration.
• Bring your insurance cards and photo ID.
