Do you prefer a male or female physician?
Do you prefer a certain age range for your physician?
Is the physician’s office conveniently located near your home or workplace?
Do their office hours work with your schedule?
Are the staff members professional and courteous?
Are you made to feel welcome?
Do staff members take time to help you with your questions?
Are the staff members helpful and accommodating when scheduling your next appointment?
Is the waiting room clean and comfortable?
Is there a place for kids to be entertained?
Has the physician ever had any disciplinary action taken against him/her?
Will the physician work with you if you have payment issues?
Is the physician taking on new patients?
How long in advance do you have to schedule appointments?
Does the physician accept your health plan?
Can the physician admit patients to your local hospital?
