During routine annual visits, providers often find patients reporting confusion about their meds as well as other medication concerns. Our specially trained pharmacists in the Medication Management Clinic are here to help.
After a referral from your provider to the Medication Management Clinic of Columbus Regional Health, our pharmacists will devote personalized time and attention to answer your medication concerns. Each visit includes a thorough review of all medications, focusing on opportunities to maximize treatment, decrease side effects and possibly reduce the number of needed medications. Patients will receive individualized education on the use of medications as well as administration tips on how to best use their medications. These visits are approximately one hour in length, and can be a one-time visit or if needed, can be multiple visits customized to meet your specific needs.
The pharmacists in the Medication Management Clinic share a passion for educating patients and families on better understanding medications and disease states to improve patient care. Whether a specific disease state, such as heart failure, hypertension, or diabetes, is the focus or simply a comprehensive review of all medications is preferred, our pharmacists will personalize your education and optimize your therapy. Any potential change or discontinuation of medication is discussed directly with your provider first, insuring a team approach to your overall health and wellbeing.
The Medication Management Clinic has been providing excellent care to patients requiring anticoagulation services since 2017. Our patients enjoy the accessible, personal care provided by our pharmacists.
