When you call for an appointment, we schedule to address all your concerns. Please help us be sure we have allowed enough time and inform the appointment desk of all your issues. We know your time is valuable to you, and we schedule accordingly.

Due to the unscheduled nature of emergencies and periods of epidemic illness, occasional delays are unavoidable. We hope that you will understand if you encounter such delays, and be comforted to know that if YOU ever require emergency attention, it will be provided.
Walk-in Clinic: Monday - Saturday, 7:30am – 9am
(no appointment needed) for established patients with acute illnesses only.

Well Child Appointments: Monday - Friday, 10am – 2:30pm (schedules vary seasonally).

Only for needs such as Injections, Vaccines, MD ordered Blood Pressure checks, Tests, and other nurse performed services.

Payment options are checks, cash, Visa, MasterCard, Discover, American Express and debit cards.
Generally, you should receive a statement every thirty (30) days unless the visit is pending with your insurance company. After forty-five (45) days, if there is no response or payment from your insurance company, the balance becomes your responsibility.
The fee schedule has been designed to represent the usual and customary charges. It reflects time spent, supplies needed and the complexity of the problem and/or procedure. This may not always agree with your insurance company's global, usual and customary determinations.
The person with whom the child/children resides with will be listed as guarantor. If someone other than the guarantor carries insurance, the insurance information will reflect this. We expect payment from the person who brings the child in for services, and we do not get involved with billing other persons or waiting for the conclusion of court cases. Our system cannot accommodate two party billing.
We realize you may have financial problems, at times, making immediate payment difficult. Please feel free to discuss this with us. While we are concerned about our collections and expense, your child’s medical care is our priority. We always try to honor a faithful relationship.
Past due amounts that are greater than 90 days over due are subject to being turned over to a collection agency. You are strongly encouraged to pay all past due amounts promptly or set up a payment plan with us.
There is a $20.00 charge for returned checks. If two checks are returned, you will no longer be able to write a check in our office. Subsequently, payments must then be made either by cash, credit card, money order, or certified check.
We participate in: Anthem, Indiana Medicaid, SIHO, Encore, Sagamore, PHCS, IHN, Aetna, Cigna, United Healthcare and Humana. A current card must be presented at every visit in order for us to file your claim. Co-pays and deductibles are due at time of service.
You must also present your insurance card for non-contracted insurances that you wish us to file on your behalf. Payment responsibility remains yours. If your insurance does not satisfy the claim within 30 days, the balance is then due from you.

The state of Indiana Law (IC 27-15-13-2 Sec 3) establishes the fees allowed to copy these records. These fees are your responsibility to pay. Insurance does not cover this service.

Established in 1974, Columbus Pediatrics has been honored to meet the newborn, pediatric and adolescent healthcare needs of families for over 37 years. Our pediatricians make up one of the largest and most experienced pediatric practices in the state. Similar philosophies on pediatric care and a commitment to excellence in medical care brought Drs. David E. Larson and Stephen R. Loheide together in 1974 to locate in a regional area in need of pediatric and indigent care. Columbus Pediatrics then grew with the additions of Dr. Stephanie Fangman in 1983, Dr. Bruce Davison in 1988, Dr. Cheryl Harris in 1989, Dr. Nancy Scott in 1991, Dr. Kathryn McAleese in 1996, Dr. Bradey Kleman and Dr. Jennifer Hartwell in 2011, and Dr Rachel Kerschner in 2013.
