Diabetes Prevention Program-Information Session

The Diabetes Prevention Program is an evidenced based program that provides a supportive environment where participants work in a small group to learn about eating healthier and increasing physical activity in order to reduce the risk for developing diabetes. 

The program goals are to reduce body weight by 5% and increase physical activity to 150 minutes per day. 

The year-long program is delivered by trained facilitators using the Prevent T2 curriculum.  The core sessions meet beginning with 16 weekly sessions and transition to core maintenance sessions that meet monthly. 

To be eligible for this program participants must be:
 - 18 years of age or older
 - Body Mass Index 25 or greater (23 or greater for Asian)
Also, at least one of the following:
  - A1c must be 5.7%-6.4% within past 12 months
  - Fasting glucose level must be 100-125 mg/dl within past 12 months
  - Previous clinical diagnosis of Gestational Diabetes Mellitus
  - Score a 9 or higher on the CDC pre-diabetes screening test
