Overview

Neurology and Sleep Sciences is located in Columbus, Indiana, with satellite offices in Seymour and Batesville.

Our team of specialists treat patients for seizure management, care of Alzheimer’s and Parkinson’s Disease, Multiple Sclerosis, carpel tunnel diagnosis, physical and stroke rehabilitation, migraines, and headaches. Additionally, Neurology and Sleep Sciences is offering a Spasticity Clinic for patients suffering from an upper motor neuron injury.

We also treat patients with sleep disorders and treat a range of conditions from sleep apnea, sleep-related seizures or limb-movement disorders, restless leg syndrome (RLS), insomnia and more.

New Patient appointments for neurological health issues require a referral from a licensed medical provider. Some examples of health issues we treat include seizure management, care of Alzheimer’s and Parkinson’s Disease patients, Multiple Sclerosis, carpel tunnel diagnosis, physical and stoke rehabilitation, migraines and headaches.

Sleep Issue patients and current patients requesting follow-up appointments may call the office to schedule an appointment.

To schedule an appointment, please call our office at 812-376-3100 extension 3334. Please briefly explain the reason for your visit. This allows us to schedule the appropriate time needed to address your healthcare concerns. For your first visit, please arrive 15 minutes prior to your appointment time so that we may obtain the information required for establishing your care.

  • Please bring the following items with you to your appointment:
    • All medications in a ziptop bag
    • All narcotic medications
    • Photo I.D.
    • Current insurance card(s)
    • Current prescription card(s)
  • Please inform the front desk staff of any change in name, address, telephone number, employment or insurance.
  • Always inform your physician of any changes in medical history.

As a courtesy to our other patients we require 24-hour advance notice of cancellation of an appointment. If you are unable to keep your appointment, please notify us as soon as possible.

Please note that for a patient that is a Minor or an Incapacitated Adult, a Parent/ Legal Guardian/Power of Attorney will need to accompany the patient and be present for every visit. If the Parent/Legal Guardian/Power of Attorney cannot be present for every visit, then they must sign a Treatment of Minor/Incapacitated Adult consent form designating an authorized agent.
