Our team of specialists treat patients for seizure management, care of Alzheimer’s and Parkinson’s Disease, Multiple Sclerosis, carpel tunnel diagnosis, physical and stroke rehabilitation, migraines, and headaches. Neurology & Sleep Sciences offers BOTOX injection treatment for patients experiencing chronic migraine, dystonia, hemifacial spasms, blephrospasm and spasticity.
We also treat patients with sleep disorders and treat a range of conditions from sleep apnea, sleep-related seizures or limb-movement disorders, restless leg syndrome (RLS), insomnia and more.

New Patient appointments for neurological health issues require a referral from a licensed medical provider. Some examples of health issues we treat include seizure management, care of Alzheimer’s and Parkinson’s Disease patients, Multiple Sclerosis, carpel tunnel diagnosis, physical and stoke rehabilitation, migraines and headaches.
Sleep Issue patients and current patients requesting follow-up appointments may call the office to schedule an appointment.
To schedule an appointment, please call our office at 812-376-3100 Option 3. Please briefly explain the reason for your visit. This allows us to schedule the appropriate time needed to address your healthcare concerns. For your first visit, please arrive 15 minutes prior to your appointment time so that we may obtain the information required for establishing your care.
As a courtesy to our other patients we require 24-hour advance notice of cancellation of an appointment. If you are unable to keep your appointment, please notify us as soon as possible.
Please note that for a patient that is a Minor or an Incapacitated Adult, a Parent/ Legal Guardian/Power of Attorney will need to accompany the patient and be present for every visit. If the Parent/Legal Guardian/Power of Attorney cannot be present for every visit, then they must sign a Treatment of Minor/Incapacitated Adult consent form designating an authorized agent.

In-Network Health Plans
Your medical records are strictly confidential. The Health Insurance Portability and Accountability Act (HIPAA) restricts us from releasing any information without your written permission.
There may be times when you request that we provide copies of our records to you or other entities. You may be billed for this service.
Our physicians review all incoming test results and this will be reviewed with you at your next scheduled appointment or over the phone.
Patients are welcome to call the office to obtain results for any test.
To obtain a medication refill, contact your local pharmacy. Please note that it may take up to 48 hours to process your request. Refills are not called in after hours or on weekends.
Please evaluate your medication supply prior to your office visit and request refills at your visit. Due to Indiana guidelines we have a new narcotic policy: all patients that obtain narcotics must sign a narcotic agreement before any narcotic prescriptions are written.
Your physician will let you know if the medication you are taking needs a written prescription.
Please notify us several working days in advance before you are in need of any narcotic prescriptions.
When picking up a prescription or sample medication from the office, a photo ID must be shown.
