The Diabetes Prevention Program provides a supportive environment where participants work together in a small group to learn about healthier eating and increasing physical activity in order to reduce their risk for developing Type 2 diabetes. The program is delivered over a 12 month period, beginning with 16 weekly sessions followed by 10 maintenance sessions.
To be eligible participants must be 18 years of age or older and have a Body Mass Index equal to or greater than 24. Participants must also meet at least one of the following requirements
- A1c must be 5.7%-6.4% within past 12 months
- Fasting glucose level must be 100-125 within past 12 months
- Previous clinical diagnosis of Gestational Diabetes Mellitus
- Score a 9 or higher on the CDC pre-diabetes screening test

DPP Eligibility Form 2017
Our class will begin on March 21, 2017 at the Mill Race Center from 6:30 pm-7:30 pm. DPP Schedule 2017-2018- Mill Race Center Attending all sessions will provide the best benefit however we do ask that a minimum of 6 sessions be attended during the first 6 months and a minumum of 4 sessions attended during the last 6 months.
Participant must also be willing to have weight recorded privately at each session and submit physical activity minutes at each session.
Pre-registration is required. The cost of the class is $260.00 to be paid in advance.
