A Team Approach to Patient Care

This summer the Columbus Regional Health Marketing team launched an initiative with the Breast Health Center to encourage screening mammograms for patients who have come to the Breast Health Center in the past but haven’t been back in over a year. This included mailing more than 5,700 reminder cards to individuals.

Outpatient Imaging Services Manager Liz Youngman said many individuals come to the Breast Health Center after receiving the reminder. One of these individuals made an appointment after missing her mammogram last year. Following her appointment at the Breast Health Center, she was called back for additional imaging and then an image-guided biopsy. The biopsy result was positive. A Nurse Navigator from the Cancer Center called with the results and set the patient up for the next Multidisciplinary Breast Health team consult. "This started with a simple card that made a tremendous difference in this patient's life," said Liz. "In six business days, this patient went from a screening mammogram to a diagnosed breast cancer along with an appointment for plan of care. We have good things happening here--we have phenomenal staff who make such a big difference in the lives we serve."

This was an excellent example of how multiple departments worked together to ensure our patients have access to appointments and prompt follow-up with a care plan, when needed--from the initial Marketing communication to the Breast Health Center, Pathology, Oncology, and the Multidisciplinary Breast Health Program.
