Completion Requirements

  • “Achieved for Residency” on at least 85% of all required goals and objectives with no active “Needs Improvement”
  • Completion of service component, which includes inpatient pharmacy staffing responsibilities
  • Completion of a year-long residency project
  • Presentation of project design and available results at the Midyear Clinical Meeting or equivalent meeting determined by the residency program
  • Presentation of project results at the Great Lakes Resident Conference (live presentation) or equivalent meeting determined by the residency program
  • Presentation of one Continuing Medical Education session (CME) to physicians and other healthcare professionals
  • Presentation of two Pharmacy Learning Sessions (PLS) to pharmacy staff
  • Completion of a DUE/MUE and presentation to the Pharmacy & Therapeutics Committee
  • Completion of a medication class review or full drug monograph for formulary addition or presentation to the Pharmacy & Therapeutics Committee
  • Completion of a policy review and presentation to the Pharmacy & Therapeutics Committee
  • Completion of a Medication Safety project
  • Attendance at 4 or more Pharmacy & Therapeutic Committee meetings during the residency year
  • Attendance at 4 or more Medication Safety Team meetings during the residency year
  • Completion of a quality improvement project
  • Participation in recruitment effects, such as ASHP Midyear Clinical Meeting Residency Showcase, IPA Pharmacy Residency Showcase, interviews, selection, and other events as determined by the program
  • Completion of residency portfolio
  • Successful completion of Basic Life Support and Advanced Cardiac Life Support curriculum
